New Patients: Download, print, and fill out these forms at home before your first appointment, or arrive 15 minutes before your scheduled time to complete the necessary paperwork. That way, we can work as efficiently as possible during our time together. We can order lab tests for you, but you are encouraged to bring any previous lab tests that are related to your area of complaint so that we can get started quickly. You can also have another doctor send your lab reports to us by mailing our Requst for Health Records form to that office. Please bring a list of your current supplements and medications, or the actual supplements and medications themselves. Confidential Patient Information Form (all new patients) note: 2-page form Pediatric Patient History (all new pediatric patients) note: 3-page form Woationrkers' Compensation Injury Report Form (if applicable) No-Fault or Personal Injury Accident Information (if applicable) note: 2-page form Request for Health Records Form which gives us permission to obtain your medical records from another office. Print and sign this form and fax to our office at (585) 442-1017 before your first appointment. All Other Patients: Print, and fill out these forms to bring to your next scheduled appointment. Patient Update form. Use this if something has changed with your medical history, you have a new injury or area of complaint, or if you have not been seen in our office within 6 months. Massage Therapy Clients: New Massage Therapy clients, or those with changes to their medical history may download, print, and bring in this Massage Medical History form, or arrive early for their next appointment to complete the necessary paperwork.
